- Please complete this Specialized Services form.
- A guidance secretary will contact you for the following documents once submitted:
- Proof of the student’s name/age/citizenship (e.g., birth certificate)
- Proof of your home address (e.g., current utility bill)
- A copy of your student’s most recent report card and IEP
- Ensure your student’s immunization record is up-to-date by checking with the Simcoe Muskoka District Health Unit. North Bay Parry Sound District Health Unit. Huntsville HS does not collect immunization information.
- After documents received a meeting will be set up with a VP, the student and parent/guardian
- A guidance counselor will contact you to discuss course selection
- After the entire registration process is complete, you will need to come to the school to sign all paperwork.
